2.Address

Mention the contact number of home and your mobile number. Also list the area code for the landline number. Employer may contact you on the mentioned numbers for any query. Hence, it is important to provide the number that is currently in use.
4.E-mail ID

E-mail ID is now widely used as a part of the contact information. All the business communication is generally done through mails. You may receive the softcopy of the documents on your mail ID and other related information. Hence do mention your E-mail ID in your personal details section.
5.Other Details

There is no need to include any other information like age ,gender , race ,nationality or marital status in personal details section . Include details like nationality or race only if asked when you are applying for government positions . Including anything other than this will make it sound ineffective and boring.
